Try your best to complete them, then email them to lunco@nssexualhealth.ca by Valentine’s Day.

To answer the questions, look through the website, especially the on-line SHIFT pamphlets. Answer as best you can – all submissions will be entered in the draw (each submission will count as one entry; one entry per email address).

All entries are confidential! The winner’s name will not be announced (unless it’s okay with the winner), and we won’t share any responses, or the fact that you entered, with anyone.
